Sample Disability Certificate

As someone is determining their eligibility to open an ABLE account based upon age of onset of disability and severity of disability, the ABLE NRC has created a Disability Certificate for your use.

This certificate is a sample of a signed physician statement.

Please note that an individual who received Social Security benefits for a disability that began before the age of 26 (or before age 46 as of 1/1/26) does not need to maintain a signed physician statement.

If you did not receive Social Security benefits, ask your physician to sign a statement to verify that your disability qualifies you to open an ABLE account. Keep this statement in your records for your future reference.

Most ABLE plans ask you to self-certify that you have a qualifying disability and do not require a form to be uploaded.
